Illness/Injury:
If a student becomes ill or is injured during the school day, he/she will report to the clinic. If the child is unable to remain at school, parents will be notified. If parents cannot be reached, the health aide will contact the people listed on the child's emergency contact form. The clinic cannot treat serious injuries or illnesses; they will apply ice as needed and keep your child as comfortable as possible until you arrive.

If the health aides feel that a child is able to stay in school but that follow-up attention may be needed, they will contact parents via phone or send a note home with the child if they are unable to contact a parent. As much as possible, notes will also routinely be sent to parents whenever their child visits the clinic or if the health aide feels your child is visiting the clinic on an unusually frequent basis.

When a child is home due to an illness, we expect that an adult will be supervising the child (Wis. State Statute 948.21). A school administrator will contact the parent or guardian of a child who is being left unsupervised at home when, in our judgment, the State Statute has been violated.

Except for unusual circumstances, we expect that when students return from an absence, they will be able to participate in all class activities including physical education, recess and other outdoor activities.

Please notify the clinic of the following: any specific health conditions or allergies, any communicable disease such as chicken pox or any immunizations received by your child throughout the year.

An adult must deliver medications to school or a parent may call the school clinic on the day that the student will bring the medication to the clinic. No medication is given to a student without completion of the medication permission form by a parent or guardian.   The medication must be in a container with the current pharmacy label on it. If students require the use of epi-pens or inhalers on a frequent basis, we strongly encourage parents to provide items that will remain in the clinic at school.   Medication forms can be obtained from the clinic; information needed on these medication forms includes: prescription number, name of medication, strength of medication, amount of pills left at the clinic and dosage/frequency of medication.

If students in grades 1-5 choose to bring any wipes/lotions to school, they should be unscented due to allergies of other students.

Medical/Dental Appointments
We suggest that medical/dental appointments be scheduled whenever possible outside of the school day. If a child does need to leave school for such an appointment, the parent needs to write a note to the classroom teacher stating the type of appointment and the time that they will be picking up the child.

For your child's safety, parents need to come to the school office at the appointed time to sign-out their child and we will summon the child from his/her classroom. Although we recognize that it may be more convenient for parents to meet students out in front of school, we believe that this practice creates a potentially dangerous situation for our students. Makeup work will be available upon the child's return to school.
