If a student becomes ill or is injured during the school day, he/she will report to the clinic. If the child is unable to remain at school, parents will be notified. If they cannot be reached, the health aide will contact the people listed on the child's emergency contact form. The clinic cannot treat serious injuries or illnesses; they will apply ice as needed and keep your child as comfortable as possible until you arrive.

When a child is home due to an illness, we expect that an adult will be supervising the child (Wis.State Statute 948.21). A school administrator will contact the parent or guardian of a child who is being left unsupervised at home when, in our judgment, the State Statute has been violated.

Except for unusual circumstances, we expect that when students return from an absence, they will be able to participate in all class activities including gym recess, and other outdoor activities.

Please notify the clinic of the following: your child's specific health condition or allergy, any communicable disease such as chicken pox or any immunizations received by your child.

Medications must be delivered to school by an adult and should not be sent with a student. No medication is given to a student without completion of the medication permission form by a parent or guardian. The medication must be in a container with the pharmacy label on it. Extra medication forms can be obtained from the clinic.

In order to have medication administered to your child at school, the Medication Administration During School form must be on file at the clinic.  We suggest that medical/dental appointments be scheduled, whenever possible, outside of the school day. If your child does need to leave school for such an appointment, the parent needs to write a note to the classroom teacher stating the type of appointment and the time that they will be picking up the child. Makeup work will be available upon the child's return to school.
For your child's safety, we expect that the parent will come to the school office at the appointed time and we will summon the child from his/her classroom. Although we recognize that it may be more convenient for parents to meet students out in front of school, we believe that this practice creates a potentially dangerous situation for our students.
